  • Retrograde Approach for a Failed Antegrade PCI: Watch Out for the Back Door

    Highlights

    • A PCI to the RCA (functional occlusion) is attempted in a patient with a previous ACS in the LAD.
    • The anterograde wire make a distal dissection with occlusion of the RCA.
    • A retrograde wire from the LAD is advanced to the RCA proximal to the stenosis, but there is an acute occlusion of the LAD.
    • Finally, three DES are implanted in the proximal-mid RCA with TIMI 3 flow but with residual distal dissection.
