  • Percutaneous Repositioning of Impella Mechanical Circulatory Support Device: Snare-Direct-Push Technique

    Highlights

    • Temporary MCS has become an essential tool in management of CS with a goal to bridge to a myocardial recovery or destination therapies.
    • Impella is increasingly utilized but not-infrequently requires adjustment of position under echo or fluoro to achieve optimal device function.
    • Impella can occasionally completely dislodge out of the left ventricle cavity into the aorta without the ability to reposition despite imaging guidance.
    • Here we describe two cases where Impella was repositioned into the LV using: Snare, direct, and push technique.
    • We were able to maintain same vascular access and Impella device, and successfully reposition the Impella device percutaneously.
