The first step when using the MANTA (Teleflex, Wayne, PA) closure device is to locate the common femoral artery access site by using the MANTA depth locator. This depth measurement can change due to operator- or procedure-related factors such as unintentional compression of the skin during depth locator measurement, or due to the accumulation of hematoma during the procedure, which ultimately increases the depth distance. In the setting of acute patient clinical deterioration such as cardiogenic shock or an acute myocardial infarction with hemodynamic instability or post-cardiac arrest, there is a need for a mechanical circulatory support device such as Impella (Abiomed, Danvers, MA) or extracorporeal membrane oxygenation (ECMO) inserted urgently. In such cases, it is often not feasible to pre-close with Perclose ProGlide (Abbott, Abbott Park, IL) or to use the depth locator from the MANTA kit. Additionally, these devices are inserted in locations such as the operating room or emergency department, where the operators do not have the skills, knowledge, or experience to appropriately use them.
