  • Editorial: Revascularization of Left Main Coronary Artery in Patients With Diabetes: Is the Pendulum Swinging to PCI?

    Diabetes is associated with accelerated atherosclerosis, diffuse coronary artery disease (CAD), more extensive disease burden, increased mortality, and higher repeat revascularization after initial revascularization strategies  . The FREEDOM (Future REvascularization Evaluation in patients with Diabetes mellitus: optimal management of Multivessel disease) trial compared percutaneous coronary intervention (PCI) versus coronary artery bypass graft (CABG) in 1900 diabetic patients with multivessel CAD but excluded left main (LM) disease  . PCI was associated with a higher mortality at 5 years follow up compared with CABG in patients with diabetes and multivessel CAD  . In the FREEDOM follow-up study, the all-cause mortality rate was significantly higher with PCI at 8 years  .
