  • Editorial: Ozone-mediated lumbar renal artery denervation – Another arrow in the quiver?

    Hypertension remains the most common cause of premature death and morbidity worldwide. Despite ubiquitous availability of safe and effective diagnostic and therapeutic tools to lower blood pressure, hypertension control rates remain unsatisfactory low [  ]. The 2023 European Society of Hypertension guidelines for the management of arterial hypertension recommend renal denervation (RDN) as an additive or alternative approach to treat high blood pressure [  ]. Radiofrequency based and ultrasound-mediated RDN are catheter-based, minimally invasive procedures, in which the renal afferent and efferent sympathetic nerve fibers are modulated (either by radiofrequency or ultrasound energy), leading to - besides others - lower sympathetic activity, thereby lowering blood pressure. RDN has been shown to be safe and effective in several randomized and sham-controlled trials, which has subsequently led to approval by the US Food and Drug Administration [  ]. Consequently, contemporary guidelines included RDN into the repertoire of blood pressure lowering therapies. Compared to other therapies in hypertension, i.e., life-style interventions, and pharmacotherapy, RDN is achieved via an invasive procedure. Although, very rare in contemporary clinical trials and real-world registries, potential associated procedure related adverse events include contrast-mediated acute kidney injury, access site complications, or renal artery injury [  ]. On the other side, RDN provides the benefit of the “always on effect” because it is independent of adherence, which strains life-style interventions and pharmacotherapy [  ]. According to current societal guidelines, radiofrequency and ultrasound mediated RDN are established as adjunct treatment approaches in the management of hypertension [  ].
