  • Editorial: Non-elective inpatient transcatheter aortic valve replacement: Satisfactory outcomes with unacceptable cost

    Transcatheter aortic valve replacement (TAVR) has evolved as the preferred choice for the treatment of severe aortic stenosis in the elderly [  ]. Imburgio et al. provided an overview of temporal trends in outcomes of non-elective inpatient TAVR from 2016 to 2020 based on Nationwide Re -admissions Database (NRD) [  ]. The study is unique in that it provided data regarding the largest cohort of inpatient (presumed non-elective) TAVR totaling 31,621 admissions. Impressively, discharge to home rates increased from 74.5 % in 2016 to 85.9 % in 2020 along with improvement in all-cause 30-day readmission rate from 14.0 % to 10.3 % respectively. The annual in-hospital mortality rate was 2.3 % without any significant change over time. The length of stay (LOS) during index hospitalization showed a nonsignificant favorable trend. The mean time from admission to TAVR was 4.6 days and mean LOS was 8.8 days which remains exceedingly long.
