1 Case description A 50-year-old premenopausal woman without cardiovascular risk factors presented with acute chest pain lasting 10 min at rest in the early morning. On presentation, the patient was stable without any symptoms. Electrocardiogram showed sinus rhythm without ST-T change and abnormal Q wave. Transthoracic echocardiography showed preserved left ventricular contraction without any wall motion abnormality. However, she was diagnosed with non-ST-elevation myocardial infarction due to elevated Troponin I levels at 0.226 ng/ml (reference range < 0.029 ng/ml).
