  • Editorial: Old CTO technique in a new slice

The parallel wire technique (PWT) was initially developed by Dr. Nicholus Reifart in Germany in the 1990s for the CTO wiring technique under the principle that once a wire has entered the CTO into a nearby distal true lumen (extra-plaque space), a second stiff wire could gain easier access through the CTO on to the distal true lumen. Since then, there have been many technological (guide wires, microcatheters, ADR devices, IVUS, DES, and antiplatelet agents) and technical (retrograde approach, ADR, algorithmic approach, and Image guidance PCI) developments in percutaneous coronary revascularization, which have contributed enormously to the high chance of procedural success with fewer complications among the dedicated operators.
