A 33 years-old woman came to our echocardiography laboratory to take an echocardiogram for systolic murmur. Patient denied syncope or symptoms in her history. The exam showed normal left ventricular internal dimensions and systolic function. No relevant valvulopathies were found, but a large, perfectly circular mass with an echo transparent lumen and thin edges was found to be implanted to the ventricular side of the anterior mitral valve leaflet (A2) ( Fig. 1 A, B, C). The mass causes a retraction of the anterior leaflet with just mild regurgitation ( Fig. 1 D). There was no evidence of mitral stenosis or left ventricular outflow tract obstruction at baseline nor during Valsalva maneouver ( Fig. 1 E and F).
