    • This case highlights the use of unconventional equipment when performing TAVR in patients with complex anatomy and an extremely calcified aortic valve (Ca score >10000).
    • No conventional catheter (or even Guideliner) would cross the valve into the LV.
    • A coronary wire and balloon inflation was required to facilitate entry into the LV.
    • Alternative techniques include the mother-in-child technique or anterograde crossing of the aortic valve via a transapical or transseptal puncture.
