  • Editorial: Refining the hemocompatibility paradigm in AMI-cardiogenic shock: Insights from an early Impella 5+ experience with DAPT and bivalirudin

Despite significant advancements in both pharmacologic and mechanical support therapies, acute myocardial infarction complicated by cardiogenic shock (AMI-CS) continues to pose significant challenges with persistently high morbidity and mortality rates. Temporary mechanical circulatory support (tMCS) devices with microaxial-flow design, such as the surgically implanted Impella 5.0 and 5.5 (Impella 5+), have reshaped management in patients presenting with advanced shock stages. These devices provide more robust hemodynamic support than earlier generations percutaneous devices (Impella 2.5 and Impella CP), and allow for prolonged left ventricular unloading, successfully bridging patients to myocardial recovery or heart replacement therapies [1].
However, the use of this technology introduces new challenges: clinicians must carefully balance the risks of thrombotic and bleeding complications, known collectively as hemocompatibility-related adverse events (HRAEs). This balance is particularly delicate in AMI-CS, where patients typically undergo primary percutaneous coronary intervention (PCI) and require both systemic anticoagulation to prevent device-related thrombotic complications and dual antiplatelet therapy (DAPT) to reduce rates of stent thrombosis.
In this issue of Cardiovascular Revascularization Medicine, a timely and thought-provoking study examined this clinical dilemma by analyzing a small but carefully characterized cohort of 13 patients with AMI-CS supported with Impella 5+ and treated concurrently with DAPT and bivalirudin [2]. By applying the recently developed Scoring Hemostasis Events and Assessment for Risk (SHEAR) score - a tool designed to objectively quantify HRAEs - the authors offer an important early glimpse into the feasibility and potential safety of this combined pharmacologic approach.
